Please note the information below on how to submit your band for a spot for Alive@Five 2011:

Anyone who would like to submit online (DSSD preferred) should do so via the following link: Note: There is a $5.00 submission fee when submitting online.

Otherwise, you can submit a hard copy for only the cost of postage.
Hard copy promo kits should include the following:
1) A completed application form (available for download from our website)
2) Demo CD (clearly labeled!) (additionally, a DVD of a recent live performance is optional)
3) Band bio
4) Band photo
5) Press clippings (if available)
6) Complete contact info:
